Award Reproduction Request Form
If you’ve lost an Award (Statement of Attainment) or Proof of Training Card, complete and return this form to request a reproduction. You can expect to hear from us within 3 business days of receiving the form.
This Trainee Resource can also be used to request an Award (Statement of Attainment) or Proof of Training Card by individuals who have changed their name and would like their documentation issued in their new name.
